Medical treatments without borders with assistance of HISCOM and e-MedSolution

The Customer

In 2011, the Hungarian "Békés County Pándy Kálmán Hospital" in Gyula and the Romanian hospital "Spitalul Clinic Judeţean de Urgenţă Arad" in Arad jointly applied for a cross-border European Union support (code number HURO/0802/021_AF, title: Medical treatments without borders in the Körös-Maros region – creating the common bases of medical care in Arad and Békés Counties).

The challenge

Within the framework of this support, T-Systems Hungary Ltd. has implemented an integrated information system. The e-MedSolution and HISCOM implementation project started in 2012. Despite that this was the ~60th e-MedSolution implementation; it was the first HISCOM implementation outside Hungary.

The objective and reason to use the two hospital’s existing information system:

  1. General objective of the project is to provide healthcare for the population of the border area, in due time, at the closest hospital from their home.
  2. The specific objectives are linked to enlarging the territory covered by joint medically assisted patients through excluding parallel analysis and improving cost effectiveness. The establishment of a common database improved the quality of both the emergency healthcare services, and the health services at all and contributed to the reduction of the socio-economic problems and deficiencies in the programme area and lead to better healthcare service in the region.
  3. The main activities of this project were connected to the development and launching of institutional information systems in Gyula and Arad, as well as an inter-institutional information system. The target group of the project is the population of the two countries along the border.

In the region many Romanians travel to Hungary and many Hungarians to Romania. Unfortunately often the travellers have to be treated in the other countries’ hospitals. Because of this, in the other country advanced registration is required, and of course from another hospital treatment history is also needed, either from the native or a foreign hospital. These events now occur in such quantity that without IT support it would be difficult to document the treatments of the patients.

The solution

In Gyula hospital, both e-MedSolution and HISCOM was used, while in Arad only HISCOM was linked to the existing Romanian clinical information system. The project started in October 2012 and ended in April 2013. During this time four months was needed to link the two systems to HISCOM.

In Gyula, the systems are implemented in seven location (the central hospital , pulmonology hospital , children's hospital , spa and rehabilitation care , Szeghalom branch and Mezőhegyes branch). These hospitals have near to 1,300 beds, while Arad owns also some 1000 beds.
In Gyula there are 870 users in 58 inpatient wards. The system is also connected to ~200 diagnostic and outpatient units.

e-MedSolution had to be further enhanced for e.g. the implementation of the equipments used in the audiology consultation hours and for the spirometer used in pulmonology.

After this the next steps were:

  • data collection in all units
  • the joint test operation of all diagnostic units
  • the joint test operation of all surgical units
  • test operation of each site or department
  • data migration from the previous HIS system
  • interfacing lung care system
  • interfacing IT lab system
  • interfacing PACS
  • interfacing diagnostic instruments

Afterwards, we have organized the training session. In small and big groups, altogether, we have trained 870 users (out of which 215 were doctors) and 3 system operators.
The exams were done by the hospitals’ information department. During the exam patient processes had to be done and the resulting patient documents were checked.
After a successful exam the users were given access rights to use e-MedSolution. In order to achieve consistent data recording, for two weeks the data recording was done by all new users in both systems.

We closed the project with live system start. For this occasion, local support was provided by T-Systems’ consultants. 10 consultants were working on site. After that, diminishing number, consultation support was provided for a week.

Arad: For the implementation of HISCOM there was no need for the development or modification of the system. The interfacing was introduced with the cooperation of the Romanian information experts.

The system operators of both hospitals were trained in using the system, and they ever since then are using it independently. T-Systems provides second level support.

The customer benefit

  • provision of healthcare delivery for the population at the closest hospital from their home independently from their country/nationality
  • enlarging the territory covered by joint medically assisted patients
  • improvement of cost effectiveness due to exclusion of parallel or multiple examinations
  • the common inter-institutional database improved the quality of healthcare services
  • increase of quality and correctness of case/treatment documentation
  • easier professional consultation.

Outlook

The result of the project proves that T-Systems is capable of:

  • the faultless implementation of HISCOM even abroad
  • the implementation of different HIS systems, even in different countries
  • to change other HIS to e-MedSolution
  • provides a bilingual forms of the patient documentation.
